This is part of the Kansas Practice Transformation Network's Compass PTN learning event Creating a Culture of Engagement and Transformation. I'll be facilitating a round table with clinics who have attempted to initiate team-based care of hypertension as part of the Blood Pressure Reduction Challenge: McPherson Medical & Surgical Associates (a nurse led care protocol), Family Health Center in Lakin (a collaborative practice agreement), and FreeState Healthcare (self-measured blood pressure readings automatically uploaded to the EHR with physician notification).
